First buy a dayrunner, or simply print out a weekly calendar from any word program, or copy the calendar' on your wall. But most importantly carry a calendar.
Set up and start using a weekly calendar that allows you to write out each hour of the day from Sunday through Saturday.
Before you fill in the calendar make a list of everything you need to do. For instance, classes, work schedule, homework, going to a birthday party, eating, showering, laundry, everything.
Now on your list assign a day to the big stuff. The little stuff start transferring them to the weekly calendar. Another words, wake up at 6:30 am, get ready from 6:30 to 8. Class at 9. Work at noon. Class at 7. Homework from 9 to 11. One day you might have something like birthday party and studying. You can also be more specific like which assignment is due.
Transfer stuff to your calendar so you can see the entire week. Now you should allow enough time to get ready, to drive places, to eat a meal, etc. Don't forget about stuff like relaxing or enjoying your favorite TV show.
Once you map out your week. You can begin to have ample time to pick out the clothes you want to wear. As for not forgetting stuff, set your stuff by the door where you leave from. Make a list of things you need to take the next day and tape it to the door at night so you see it before you walk out that day.
These are all ideas that should help you learn to manage your time and tidbits to organizing yourself.
As time moves on you will learn to be more efficient at mapping out your week and organizing your life.
PS: Don't forget to enjoy your life, even if you have to schedule time for that!!!!

The good news is that you recognize a problem and you want to fix it.
Don't wait. Make yourself a weekly schedule.
Write down everything that you need to do, and when you need to do it.
If you get up late, go buy an alarm clock and put it away from your bed so that you have to get up to turn if off.
If you cannot decide what to wear, every Sunday, get your clothes ready for the week. Put them in order so that you don't have to think about it again.
Make a list of the things you need to take to work or school each day, and check it before you leave home. If you can make this list ahead of time, say once a week, that would be best. Perhaps you can make a master list and reuse it day after day, or week after week.
If you set aside time one day a week to make a plan for the rest of the week, your life will be much simpler and you will not waste time thinking about what to wear, what you need to have with you, what you need to do.

Get a schedule book or calendar and write everything down. Check the schedule everyday to see what needs to be done. Get an alarm clock and set it to wake you up every morning. You must learn to be responsible so you will grow up to be a good citizen, get good grades and a good job.
