Yes, coolant wil turn the trans fluid milky. Most auto trans use a cooling coil in the bottom tank of the radiator to cool the tranny fluid. The fluid goes thru lines to the cooler, thru the cooler, then the fluid goes back in the tranny. If there is a serious amount of coolant in trans fluid, replace the radiator as well as has the tranny and lines flushed.

Coolant should not be-able to mix with trans fluid, oil on the other hand could and would be milky.

Yes... check your radiator to see if there is any transmission fluid in it as well...

Yes. Water condensation can cause this too. Get it flushed and filled.
