No, it's not (sorry everyone above). It's a spot foul up to 15 yards, but only 15 if the spot is further than 15 yards from the line of scrimmage.

Yes, unless the play starts within 15 yards of the goal, then it's half the distance, or if in the end zone the ball is spotted on the 2.

yes - except inside the 30 - then it's halfway to the goal or on the one yard line if it happens in the end zone.
