A singing telegram.
I have no idea, where I am from to find a person to do that. The only people who sing here, are at restaurants when it is your supposed birthday.
Hm mm.. Do you have a church group or youth group that could visit him? That would be nice. Have the children make cards for him to wish him a fast recovery.
If this is not sounding good, I would get him something that would be really pretty and something that he likes. You could get him a cool lamp or something really wild that lights up. If he likes music, give him something to listen to with headphones. If he likes to read, get him a book. Does he have a way to call people without costing him a fortune? Loan him a cell phone or give him a calling card.
If I were in the hospital, I would like cards and flowers as well as anything else. Plus some great food from my favorite restaurants, depending on my condition.
I hope your brother gets better soon. Just get him something that he is interested in or something bright to cheer him up. You know him well, so think about what he likes or what you think he would appreciate.

If he didn't have an operation on his feet, you could get him reflexology socks and top it off with a reflexology session.
Very easy to follow, if you don't know how to do it!
Also very relaxing for him, I would imagine.
