If you are OUT OF the USA, then you can probably get anyone of the many VOIP services. Try http://www.skype.com first.
If you know someone in the USA, then can get you a Vonage Adapter from a local store. Plug this into your High Speed Internet and activate the service with a USA credit card. I have used my Vonage Adapter quite successfully while visiting in Hong Kong.
Many people that have friends and family in the USA will get this service for you. Check out different packages.

There are various bits of kit you could use. You could either use a microphone and speakers, headphones and a mic and now there are telephones that plug into a space usb socket that are quite good.
Also, you may want to consider using a webcam.

use skype. go to skype.com and click get it now. as long as you have a headset or a microphone and a speaker, you're good. it's free too. and if you have the skype conference phone.
