f'(140) means: how does the dose change if the weight of the patient increases or decreases?

If you plot D versus W in a graph -- the weight horizontal, the height vertical -- the graph will be some curve. Look up the point belonging to 140 pounds. You can draw the tangent line to that point (line in the same direction as the curve there). The slope of that line is equal to f'(140). It is measured in milligrams of substance per pound of weight.
