It's all colonial. England had colonies thorughout the world. The ones in North America were the American colonies, and colonials form what is now the United States were referred o as "Americans," as opposed to "Australians" or "South Africans". Canadians were Americans until the U.S. was formed, and "American" became a dirty word in Britain. So loyalists were termed Canadians, and the splitters, "Americans", were left to be so. And so it has remained.

As an American (yes, I know I just said that, I'll get back to it in a sec) living abroad, I always make an effort to introduce myself as 'from the United States' because America is a lot more than the USA.

However if you look at how I started this answer, you'll see that it would have been really awkward and long to say "as a citizen of the United States of America" and so in some cases sheer laziness and the fact that everyone knows what I am talking about when I say "I'm American" lends itself to using this (incorrect) terminology.

I think USA was more prominent in the world's news for so long it became easier to say it that way just for the fewer number of syllables needed, - it's longer to say" The United States of America".
