I'm trying to locate Dr Peck, specializing in gynocology, he may have moved or retired...He was located downtown Oakville close to Trafalgar Rd about 8 to 10 years ago. He also resided in Oakville at the time. Or who has taken over his practice, I am looking for all my medical files as it included my details regarding my uterian cancer and endometrious. Thankyou for anything you can find out for me...Toni

I'd visit the College of Physicians and Surgeons of Ontario website to see whether he is still actively practicing or not (http://www.cpso.on.ca; go to doctor search --> advanced search and fill out the required fields (just first and last name (include hyphen if it's "richmond-peck"), gender; be sure to indicate that you are looking for all physicians (active and inactive)). Look at the summary and other tabs for information. If there's no practice address, note the registration number. You might want to just call the College directly, explain that you're a former patient, provide the reg. number and names and ask them if they have a forwarding address/contact info or whether someone's taken over his practice. When my doctor moved out of town, I received a letter from Records Storage and Retrieval Services in Toronto (1-888-563-3732). He had turned his patient files over to this service (tons of doctors do when they close their practices, apparently; maybe a letter like this was mailed to your previous address?). The storage service is required to keep patient information on (confidential) file for a specific period of time before it is destroyed in order to accomodate inquiries.They informed me that I could order my records in hard copy or on CD for a set fee and I received them last year. As a last resort, I'd try to contact any labs where you remember having tests done or the Medical Records office of any hospitals where you received treatment. He would have had to supply your specialist with test results and a detailed referrral note at the time.

Contact the Ontario Ministry of Health and I'll bet they can find right where he is.

Try calling Oakville Trafalgar Hospital they may know where his is or who has taken over his practice.
