I am not an expert. But if I had to take a guess: Early mamalian males tend to almost force the female into sex, such is with cats. The female wants sex, and calls for it, but knows that in the case of cats it is so painful that they don't actually want it when a male comes around. Thus male cats actually hold the female by the neck during intercourse to prevent them from running away. There are other instances of this in the wild.

Also, it is well documented that the pain and pleasure centers of the brain are so closely related that they sometimes confuse signals, and there is even a medical condition where the two get reveresed where things that should be painful give the individual great pleasure and vice versa.

So in the cases of heightened excitement, even for normal people, small amounts of pain may be perceived as very pleasureable.
