If she can speak any other language fluently, then it cannot be health problem but only due to lack of practise of speaking in tamil. As any other language, reading and writing is not a guarantee that one speak fluently in that language. Written and spoken form of tamil vary greatly; so one cannot learn to speak tamil by textbooks. Also, if one speaks the high form of tamil found in text books, in everyday life it becomes funny. To learn and familiarise herself with spoken tamil, she can watch tamil movies and televisions and also listen to tamil radio channels. Such channels are available online too. Above all she needs to practise it with someone who can speak fluently and mingle with tamil speaking circles. Lack of practise may be because she was brough up in a non-tamil speaking country or state. Else, her parents, family and relatives might not have had the habit of speaking tamil in home and she might have been educated in a strictly no-tamil school. It is also possible that some might very well know to speak in tamil and yet hide the fact to project themselves as "modern" (?) and fashionable among their friends. Finally, to learn spoken tamil she should have the need, motivation, interest and the effort. Otherwise, no one can learn any language.

Your question is either biased or ignorant. you ought to have said whether she is able to converse correctly in any other language. in case she can, nothing wrong. fluency in language is only due to usage. in the new india that has emerged there are many tamilians in bihar, who may not be able to speak tamil fluently. but their hindi and english are fine.
