The underlying cause is always power greed.
The way to get the armies together and keep them fighting includes both nationalism (not always blind) and religious fanaticism but also force from the rulers.

I don't think it is blind nationalism as many wars are nationally internal war; race wars, north and south wars.

I would have to say religious fanatacism. Look at the war going on now. Even though they don't say it, it still has undertones of religion. Bush wouldn't dare attack someone that's Christian, but it's okay because they are Muslim.

Religion is part of nationalism.
The bloodiest wars occured and were raised by nationalism (i.e. WWI, WWII). However, a number of wars were raised for stupid ethnic reasons (i.e. the genocides of several african nations) and more commonly for land (i.e. The Mexican-American War, French and Indian War, and many revolutions and civil wars).
