In Britian during WW2 people talked about "Adolf", not as we do today when we refer to him as Hitler.
It is an interesting question. I think that the relative commonness of his surname does have a lot to do with it.
Idi Amin, for example, was always referred to as Idi Amin. It's a common surname.
GW Bush is referred to as Dubya.
Saddam backward sounds like "Mad as...". Which may have something to do with it? His name also sounds like So Damn Insane.

I just think it is because he is an enemy we do not give him the formal respect of addressing him - (ex) President Hussein or even now Mr Hussein.
It is a formal/familiar thing, we are using the familiar and not giving him the formal respect. I think it started during the Gulf War.

you are probably right on both counts, but it also may be true that Sadam has a harsher sound. It is harder to say Hussein with emphasis.
