A Picture With A Twist
I'm gonna draw a picture.
A picture with a twist.
I'll draw it with a razor blade
And carve it in my wrist.
If I dont do it correctly
A fountain will appear.
Draining all my sorrow,
Drowning all my fear.

That seem to be the whole poem, but you can change the ending. So I think the ending is whatever you make it.
I'm going to draw a picture
A picture with a twist
I"ll draw it with a razor
I'll draw it on my wrist
And if i draw correctly
A fountain should appear
Draining all my sorrow
Draining all my fear
I’m going to draw a picture
A picture with a twist
I’ll draw it with a razor
I’ll draw it on my wrist
And a red fountain will flow
And as the fountain flows my troubles go
Im going to draw a picture

This one seems to get around.......many different versions and many people trying to take credit for it.
It looks like people like the basic poem so much that they keep adding to it and rewriting it.
Here's the longest version I found so far:
Even with wings
I cannot fly
My world gets smaller
As time passes by
His cruel intentions
Have engaged in my head
Too many things to worry about
So many things left unsaid
So now Im going to draw a picture
A picture with a twist
I'll draw it with a razor blade
I'll draw it on my wrist
As I draw this picture
A fountain will appear
As I am pleased by the sweet release
My problems will dissapear
He knows not what I do
Or what I have done in the past
Soon my life will slip away
I better get to him fast
Even though he is not there
Even though we are through
I know that he cares about me
And I still care about him too

Thats actually the whole poem but i also found this version

So ive searched a million sites and alot of peeps know the poem but dont know who its by but it might be bye samantha R.

after the doctors show,
a picture on paper there will be
of the pain and misery
that makes me write
this type of poetry.
Instead of razor, go with rubberband.
