If you have a buddy on your YM list which is online, click on it's name to chat with it. Then on the top-left corner, click on call to call the other person. Calls needs mircophones, so you and your YM buddy must have a mircophone.

Send an Instant Message or make a PC-to-PC call.
1. Roll your mouse (don't click) over the name of a person in your Messenger List. A contact card will pop up with the person's name.
2. Click on the IM or Call icon to instantly connect.
As the others have said, you need a microphone and headphone/speakers.

Set it up from YM. You then can talk to someone on at the same time with a headphone+microphone which hook to the microphone jack, and the headphone jack.
It's fun. If you have a PC camera, and the other one also has, it's even more fun.
