Most people close off their throat when they sing. Make sure the back of your tongue is down, not up.

get a GOOD voice teacher. practice all you want, but you could be hurting and not helping which is why a voice teacher is key. they teach you to breathe so don't worry about that. just make sure you let your teacher know what type of music you'd like to sing. also ask if you can watch a few lessons before you start, this way you can check out the teacher and get an idea of what you need to do. not to mention your getting cheap if not free indfromation

A lot of practice. But a voice instructor (singing teacher) or whatever would benefit anyone a lot. Also breathing techniques are key.
So I would say taking vocal lessons is the best way about it. Or taking choir at school.

If you can't get a vocal instructor, opening your mouth wider helps when singing higher notes.
But getting a vocal instructor is your best bet.
