this question does depend on your definition of madness. A customer/friend asked me this question tonight in the video shop where I work, so I am asking on behalf on someone else and cannot be specific as to the type of madness.

One Flew Over the Cuckoo's Nest (1975)
Directed by Miloš Forman.

This movie was the first to win all five major Academy Awards since It Happened One Night in 1934. This accomplishment was not repeated until 1991, with The Silence of the Lambs.

Highly recommend movie. Try and watch it if you have not seen it yet. And I’m sure this movie will be suggested by many more below.

The Fisher King, with Robin Williams. A poignant and toutchingly beautiful portrayal of madness and how someone might go mad. Utterly worth watching on every level.

Gothika is a great film, well worth seeing on the mad front. But if you want a more innocent film more touching than frightning you could go for James Stewart in HARVEY, you know the one, has a friend (six foot rabbit). Very good sweet film.

Crime and Punishment, it's also a very good book, but if you're not a big reader it's also a good film. It's about a poor man living in Russia who commits a murder and slowly loses a grip on reality.
If you're a reader try Catcher in the rye and
Zen and the art of motorcycle maintenance
they are very good
