I use Jasco Floor Patch & Leveler. It is very easy to apply and clean up is a snap with warm water. I use it at work with plenty of heavy forklift traffic and it holds up well under duress. You need to apply it when the temp is above 50 degrees. It is available at Home Depot. Runs about $20. I applied some to my drive last tear and it handle a Michigan Winter well with little wear so far.

when you say badly cracked that can mean many things, and most repairs mentioned are for small cracks. I am assuming you have a major set of cracks. if so just bring in a load of concrete and repour....

Caulk the cracks using self leveling Urethane caulk or remove it and repour the slab.

use any hydraulic water stop. it can be made runny,so to infiltrate the crack,and it is expanding as it dries.when it dries and expands it will apply atight bond
