Saying "um" and "like" is usually an unconscious way of stalling for time to think of what you're going to say next.
To stop saying these things, you need to think about not doing it while you're talking. You may feel awkward at first, but after a bit of concentration, you'll be able to break the habit.
It helps if you can ask a friend to monitor your "ums" and "likes" in conversation and tell you about them. Sometimes they just come out -- and you, um, don't even notice, like, you know what I'm saying?

Usually when you start saying umm and like, it's probably you are trying to think of what to say as you go because you may be pressured for a response but you have nothing to say so you result in saying um and like. My suggestion is to speak slower (especially as a Northeasterner, id know).
Notice how teachers tend to speak slow to get the point across whereas when people talk fast, you end up catching only the key words instead of listening to every word?
Do not be afraid to speak a little slower.
Also, think out the entire idea before you say it, the structure that you want to present an idea, even in a regular conversation.
lastly, being consciously aware and it will take time to achieve what you want so be patient. GOOD LUCK!!
P.S. It is better to keep your mouth closed and let people think you are a fool than to open it and remove all doubt.

Find a Toastmasters club and join. They're past masters at taking the ums, aws, gees, and likes out of your vocabulary.

Think before you answer. People these days are always so quick to respond. It's a conversation, not a game show.
A good stalling tactic is to say "Hmm, how interesting...", then pause and give a fully thought out answer. You might need to practice.

Don't know about normal conversations, but if you want to reduce them in public speaking, it helps to practice the speech as much as you can.

put a rubber band around your hand and every time you say um or like, snap the rubber band so you associate something painful and those words.
