A PAP smear is only to test for abnormalities in the cervical structure (to look for abnormal cells that may indicate HPV infection or the beginnings of something that may need to be treated).

If you think you might have an STD or are just interested in making sure you don't, you need to speak up and ask for a screening.
Most docs will not perform any screening tests unless you ask for them or unless they detect an issue that needs further investigation (discharge, irritation to the tissues, etc), or unless that is the specific reason for your visit - that you're there to have symptoms assessed.

HPV will show up as abnormal cells on the PAP. If they find any, they'll do a biopsy. Usually, they will also scrap for chlamydia and gonorrhea, especially at a clinic. Other than that they'll do a blood test.

You should request that it be done at the same time but I don't think they just do it without you asking.
