The drainage may have been a result of the packing that was left inside. Sloppy. An X-ray could be helpful to make sure that nothing else is there. His parents must continue the antibiotics until completed. They can continue to monitor him and observe for signs and symptoms of infection: Elevated temp; nausea/vomiting; pain, redness, swelling and heat at the incision site, yellow/yellow-green/green-blue drainage with a foul smell or bloody drainage. If the drainage is clear or pink, that's okay. Also monitor for dehiscense which is a reopening of the incision. Your nephew's parents can contact his surgeon if they have any questions or concerns.
I don't understand the RN who says "IF the wound was deep.." The boy had an appendectomy. There's no "if".

Firstly and most importantly seek medical assistance scans etc.. then think about taking legal action..
It could be infection from the dressing left inside or there could still be some dressing inside...

if the wound was deep it is likely healing from the inside out, as long as the drainage is clear I wouldn't worry too much. I would keep it covered with a sterile dressing until the wound has healed closed.

See a solicitor and SUE. The hospital was negligent and so were the operating room staff. And if still in doubt, see your GP or go back to hospital. At least it would put your mind(s) at ease.
Good luck and hope he gets well soon.
