It means 4 times a day, and is generally used for taking medications. It does not mean every 6 hours as another person answered. In that case, the abbreviation would be q6h. Generally QID is used for medicines where the time the medication is taken is not critical, as opposed to antibiotics, some cardiac medications and medications for Parkinson's disease.
