Hellraiser and the Freddie movies kept me awake when I was little. The Exorcist is really scary, I've never seen it completely because I think it will stick to me somehow.

I was gonna say the Usual Suspects, but somebody already beat me to it. Sigh.
My other choice would be the movie where Ed Norton played a young killer who murdered a priest, and Richard Gere was the lawyer defending him. Don't remember the name right now.

Hands down, "Silence of the Lambs"
It won the Best Picture, Best Director, Best Actor, Best Actress and Best Screenplay.
It followed the book precisely and was superbly acted.
