I'm writing an e-mail to a general manager who has a Dr's title. How should I start the e-mail: Dear Mrs Dr. X
In the attention of Mrs Dr. X
In the attention of Dr.X ???

if this is a formal letter just start it with

Dear Dr. X,

or just plain,

Dr. X,

I never assume they're married and she already knows that she's a doctor.
Althought it's entirely up to you I woud just say Dear Ms X
