Major League Baseball contracts it out to a private company, usually choosing among multiple bids. From there, the company takes requests and/or special needs from teams about that particular season's schedule, then gets to work on it.

Once completed, the schedule is sent to the Players' Union for approval, then it's given to MLB and its teams.
