The FIA and Italian authorities still maintain that Senna was not killed instantly, but rather died in hospital, to where he had been rushed by helicopter after an emergency tracheotomy and IV administration were performed. There is an ongoing debate as to why Senna was not declared dead at the track. Under Italian law when a person dies at a sporting event, that death must be investigated, causing the sporting event to be cancelled.
The FIA dismisses that conception as an unfounded conspiracy theory.

From memory i seem to remember it was a technical fault. I'm not mechanic but seem to remember it being reported that the shaft that connects his steering wheel to the front wheels snapped as he turned into the corner at the end of the straight, this resulted in Ayrton Senna and his car carrying straight on into the wall as opposed to going around the corner.

They couldn't prove that the steering column snapped before he hit the wall, so it's now believed that that was a consequence of the impact. It is now generally thought that the car bottomed out and careered off the track as a result.
It was an accident, no one's fault.

It was purely bad luck. Whether the car bottomed out, or the steering column snapped, it wasn't a life-threatening crash - however, when the suspension broke and a piece pierced his helmet, that was it.
