Present perfect is with "have" (have, has, 've, 's).
Past perfect is with "had" ('d).

Past perfect always refers to an action that took place BEFORE another action that was also in the past.
Present perfect can be an action that started in the past and continues to the present, or that happened at an unspecified time in the past.
