You need a period at the end, plus if the subject "we" is referring to more than one person the "name" should be plural- "names." If "we" refers to an organization or business then your sentence is correct, just add that period.

Well, it's not incorrect, but I would word it differently. Try this...
"Please be informed that our name has recently changed." or "We would like to inform you that we have recently changed our name."
