An autobiography is something someone writes about himself. So you should use the word "I" and not "he."
You might want to start out with something more interesting than saying
"I was born on such and such a date."
If you're in the 9th grade, you're probably 15 or 16 years old. That means you were born around 1990 or 1991. Maybe you'd want to start of with something about that. For example,
"I was born before the Internet, before e-mail, before most homes used personal computers as much as they use refrigerators -- way back in 1990. Since then, the world has come a long way, and so have I."
Then you can talk about your life, a little, and go up to your near your death. Since it's an autobiography, you can't really write about *your own* death. Maybe you could add an editor's note, written by a son you had as a young man. Something like:
"Editor's note: My father, Joe "Joey" Mallon, did a lot of wonderful things in his life, including raising me. He started declining a bit around his 105th birthday, and died at 107. Signed, Joe Mallon, Jr."
If I were your teacher, I'd love it.

Joey - An autobiography is the life story of a person and written by that person. Therefore, you need to write it as if YOU are writing about yourself. The second way that you started is correct. However, I wouldn't say in the first sentence, how you die. Just write from the time you were born, and then sequential things that have happened in your life. For example, My name is Joey Mallon. I was born on ___ in the city of __. My parents are ___. When I was one year old, I ____. I took a trip to ___ when I was ___ years old.
Joey - give specifics about some of the things that have happened in your life.

Since it's an autobiography, you don't have to say I, Joey Mallon. Just 'I' will do.
I was born on, and my parents, XXX and XXX Mallon, happily welcomed their (first son, second child, fourteenth kid or whatever). Before long I learned to walk, and to talk. (here's where you fill in a few details and a funny anecdote if you can)
When I reached the ripe old age of six, they turned me loose on the XXX school system. (remember something about your early days in school, or mention your favorite sports or whatever.
Bring it up to your present age, and then you can take off into the wonderful world of fantasy.

I think you should say that like this one: I am Cindy Harp, 16 years old and I live in (the address). I was born on February 12, 1990. My father is Mr. Danilo Harp and a hotel manager. While my mother is Mrs. Linda Harp who is an accountant.
This is only for the first pargraph..It should be more about your self, your interests and others...In the next paragraph, your educational background..Then at the last your dreams, ambitions or goals with the ways of achieving it...

My name is Joey Mallon and I was born on month,day, year. My parents names are Jane and Josh. I have this many siblings and their ages are ? ? ?. My grandparents names are ? and ? and they live in ?. I live in Somewhere, IL and we lived there for ? years. I went to pre-school at ? and then went to ? up until now.
You can also add what you and your family enjoy and what activities and/or sports you play and have played. Stuff like that.

Write it in the first person, since it's an autobiography: I was born on... My parents were... I died on...
I recommend you try to start it a little different from the usual "I was born on..." Maybe describe the day or the location or the hospital first: On a bright spring morning, while others were hustling to work, my mother was struggling in the delivery room.

What is your earliest memory?
Have you ever moved?
What profession/s do your parents have?
What is their education level?
Are your grandparents still living?
What is your ancestry (what countries did your great great grandparents come from)?
What sports do you like to play/to watch?
What skills do you have that you feel you are good at?
What are your favorite subjects at school?
What profession/s do you think you might like to try?
What are your favorite television shows and why?
I could go on, but I think we are well past one page...

as an autobiography you should be allowed 'literary licence'...that means that YOU get to decide when YOU are the author. I would choose the 1st person (that is, saying "I was born"). If I were doing a regular biography (on someone else of course), I'd use "John Smith was born...".

An autobiography, traditionally, is written in 1st person (I, me, my), but your teacher shouldn't count off if you had a creative angle on the assignment, AKA use 3rd person (He, his).
Just write it in whatever way you feel comfortable with and confident about.
