cash is good, but its always more fun to get a gift card to a store you know has things he likes. If you give cash, he might spend it on random junk, food, etc... but if you give him a best buy gift card, he'll for sure use it to buy some video game, cd, or electronic device he's been wanting to get but hasn't been able to convince him self to spend the cash on it.
He won't even realize it, but he'll be much more thankful you got him a gift card instead of cash.

Depends on how old he is and what kind of relationship you have. If you're concerned putting something like "Love," before your name, just sign your name. Or are you concerned what to physically put in it (i.e. money...it's hard to tell from your request). If you feel close enough to him, and you are secure in your manliness, then sign it "Love," followed by your name. If not, write something like "Happy Birthday."
