Look in to your state university Med school, or see if your area has a teaching hospital. Like Boston has Tufts Univ, Tufts Med Sch, and Tufts Univ Med Ctr, Tufts Dentistry School, and so forth (example) Still might not be free, but costs are usually minimal

If you can prove that you don't have the money for the surgery, and it's necessary to sustain your life, then some surgeons will perform the surgery pro bono. I don't think there's a list of surgeons that do this.

I don't think that there is any such animal. Maybe you could apply for medical assistance in your state?
