A blank music book so he can write his own songs in it! Pair it with a cool embroidered guitar strap and some picks. :-)

Well, if you know or can find out what his favourite band is, you could get him a music book (i.e. the notes for songs of that band). They are usually available at all music stores that sell guitars. Or, you could get him a cool-looking shoulder strap, i.e. the strap used to hold the guitar while playing standing up - there should be some nice ones out there.
Also, perhaps a guitar case, though I guess he has one already.
Or, an electronic guitar tuner that making tuning so much easier, again available at any guitar store. Those come in handy all the time.

How about a strap button set? You can get them in different designs and they run about $15.00. They are easy to put on and do not require any damage or modification to the guitar. Just a way to personalize his guitar a bit.
Or a guitar tuner.... anywhere from 7 to 50 dollars.
Or a capo for 7 to 40 dollars.
Or a new strap, from $5 to $30 and they have some terrific ones out there now. I saw one that looks like a police crime scene tape!
A peg winder is a great gift and very inexpensive.
A guitar stand... anywhere from $20.00 to $50.00
There are lots of things you can get him. Since I did not know if he has an accoustic or electric guitar, I kept everything generic.

some of those things that you use to play the guitar when your nails are short (don't know the word in english). Or strings for the guitar. Cables are never bad if you constantly play live because the get easily screwed up.

Go to a music store and find out about the different pedals you can get, a stand to hold his music books, a book of his favouite bands tunes, strings, picks- there's tons of things you'll find

You can buy little packs of guitar picks. I'd say get a bag of those and decorate them with paint or with Sharpies to say cute things, or have cute pictures on them. Then, whenever he plays, he'll be holding you gift. He'll love it, trust me.
