Normally, I would not answer a question like this as my expertise is generally in medication but I had severe sciatica while I was pregnant with my 3rd child (after having had twins) and I had sciatica so bad that I was finding it difficult to walk. I tried massage therapy and physical therapy and could not take medication
I went to the chiropractor and the pain was instantly relieved. My hips and lower back were out of alignment because I had carried twins the year before. On occasion a Doctor of Osteopathy (DO) can also work but they usually only work on one joint at a time - the chiropractor will do your whole back which works faster. Anyone who does not believe in chiropractors will tell you they are quacks - don't listen.
I don't necessarily believe that chiropractors can cure things like diabetes but they can definitely relieve some types of back pain especially if a pinched nerve is involved which is usually the cause of sciatica. Try a reputable chiropractor ASAP!

Does it hurts when the environment is cold? If yes, there is something wrong with your lower bones such as your lumbar. Since you had an accident, you should go to a doctor a have an x-ray. Medications will only help you relieve the pain, but it will keep on coming.

i was in 3 accidents back around 1995 and i was rear-ended in all 3.......since then, it took about 10 yrs to get where i could hardly walk....anyway, in 2001 i got on Medicare Disability b/c i went to the doctor and found out i had severe scoliosis...lower back pain.......last yr. in 2005 i had to have 2 major back operations..they finally ended up fusing my lumber (lower back) and i'm still in alot of pain....i guess what i'm trying to tell you is that even though your accident was 4 yrs. ago sometimes it takes that long or longer for it to get worse and worse.....i should have sued those people who hit me but i ended up leaving the scene of the accident (DUMB MOVE) and now i have really not been compensated for all the stress, PAIN not to mention medical bills that i have now. GO TO A Specialist ASAP before you get worse!!!!!!!!!!!!!!!!!!!!!!!!!!!!

so you've had the xrays and such? You have had ythe physical therapy? You sued the dude for pain and suffering? Hopefully, because you are primed for back surgery, but the longer you wait, the worse things get.

try a tens machine. I suffer chronic back pain due to a degenrative spinal disease and my tens machine is brilliant. Your doctor will have one at the surgery which you can ask to try out, then you can buy your own cheaply from ebay or boots.
