You have to read your lease agreement to see if there are any short term cancellation provisions.
You will also have to read your lease to see if there are any repair work provisions. Usually, the tenant has to put the landlord on written notice if there is a problem, then give adequate time for him to do repairs. If the repair work is not done, you should have the repairs done then charge the land lord for the work, as long as it is the landlord's responsibility in the lease.
You have not listed anything that would be grounds for unilateral cancellation of the lease.

Contact an attorney, if you already haven't, that deals with tenant issues.
Also, contact the Tenants Union in your area and get info on your rights as a tenant. Even the Housing Urban Development; HUD, and file a complaint. Contact the Attorney Generals Office in your state as well.
The fact that they sealed your sliding glass door is illegal. If there was a fire and you could not get out of the main entrance, what are you supposed to do. It wouldn't hurt to contact the Fire Department in your city to let them be aware of the sliding glass door being sealed.

Call in the inspectors from the Fire Department. That sealed sliding glass door will get them into SSSSSOOOO MUCH trouble!
The leasing agent still has to meet safety standards by law. Not doing so is a breach of contract, and is probably enough reason for you to leave without penalty.

Um....the sliding glass door thing could be a major safety hazard.
Document all complaints with dates, times, etc. Take pictures. THen draft a letter with all your backup and ask that they allow you out of your lease because they are not holding up their end of the deal. Its inexcusable to tell you to break a double pained glass with a fire extinguisher!!
Worse comes to worse, tell them you'll report the safety violation if they dont let you out.

They "said" that they would fix up the place (carpet, sliding glass door etc). Did you get that in writting? If so, then you should be able to get out of your lease. If not then that may be a little more difficult. Good luck! These things can get sticky!

It would be much easier for you to get out of it if you had someone lined up to replace you. That's how the people who lived in the apartment that we live in before us got out of their lease.

See a lawyer. They can draft a threatening letter and get you out of your lease.

if they broke their part of the lease (something listed in writing and provable) you have chance...also make sure all your complaints are in writing and you keep a copy
