Go to the State of Texas' website (I don't know what it is) Then go to their department of revenue. They will have a form online you can fill out to apply for one.

just go to the county tax assessor.
provide your social security number and $10.00

ive got one till 2008. thats all it costs.

oh, and the name of your business.
