In terms of the overall concept, including the amazing album cover, I would say Thick as a Brick, by Jethro Tull.

Hmm.... Ever? That's hard to say, for certain. However, I can think of several albums that were pretty ambitious, for their time.

Yes - "Close To The Edge" and "Tales From Topographic Oceans"

Genesis - "Nursury Cryme" and "Foxtrot"

Rush - "Caress Of Steel" and "Hemispheres"

Dream Theater - "Scenes From A Memory"

The Beatles - "Sgt. Pepper's Lonely Hearts Club Band"

King Crimson - "In The Court Of The Crimson King"

Frank Zappa - Basically anything he did was fairly ambitous.

Eric Johnson - "Venus Isle"

The Mars Volta - "De-loused In The Comatorium"

Emerson, Lake & Palmer - "Brain Salad Surgery"

The Flower Kings - "Stardust We Are"

That's tough; I'd choose either "See the Light" by Jeff Healey or "Layla and other assorted love songs" by Derek and the Dominos. Why?

"See the Light" is the debut album by the Jeff Healey Band and he plays the guitar in a way perhaps nobody else has; it just happens that he is blind. His songs range from romantic country sounding "Angel Eyes" to hardcore instrumental-vocal-blues "See the Light". His followup albums all have something to offer though "Feel This" was the hardest one for me to accept.
To see him in concert, which I have, is amazing!

"Layla..." was ambitious because Eric Clapton tried to front an "anonymous band" and he produced the extraordinary "Layla" with other bluesy songs...
