Our house has no exhaust fan in the upstairs bathroom as it originally had no shower (just bathtub). If I place a small fan on the shelf above the sink, and leave the door open will this get the vapour out of the bathroom sufficiently?

Yes.. it could be used as such.. they are not as efficient but.. still could be used... You could buy a small one and attach it to the wall on the inside and instead of having it blowing in your face, you could face it towards the ceiling and it would have a good effect

Above the sink?

Only if there is no water in the sink, especially while you are using the sink containing water.
Sh** happens.

It will ventilate better if there is a window to open and draw fresh air, or use the fan as an exhaust to blow air outside.

Dont you have a window in the bathroom?... I really wouldnt use a portable fan in the bathroom. while your using it, maybe after your done in there.

It would work, but then you have the problem of putting all that moisture into the rest of your house.
