You find your local social security office. All you have to fill out is an application show proof of your marriage and your birth certificate for proof of citizenship then they will send you a new card.

To change your name on your Social Security card:

Complete an Application For A Social Security Card (Form SS-5);
Show us proof of your:
U.S. citizenship (if you have not previously established your citizenship with us) or immigration status;
Legal name change; and
Identity.
Take (or mail) your completed application and documents to your local Social Security office.

I just got married 3 months ago. SS is national, not just for Utah, so you can call their 800 number or go to the local administration office & get a form for a new card. It's free. You just fill out the form & either mail it in or take it in along with your marriage license & they'll send you a new card along with your license back. Good luck!

Social Security has nothing to do with specific states, it's a federal matter. When you applied for your marriage license, they should have given you paperwork to fill out and send in. If they didn't, go to your local Social Security office and tell them what's up.
