There should be options on the tamogochi. When you click on the buttons to select options, there would be pictures/symbols to represent each activity. The doctor one either looks like a syringe, or a happy face with some band around his head. For cleaning, there should be either a brush and pan, or a toilet. Hopefully this helps =) and after you do that, feed the tamogochi!

What type of Tomagachi is it? If you can tell me, I might be able to help. As least for the poop, if its a dog, let it outside(if it has to go) otherwise clean up the poop...And for the poison, try giving it a piece of food or medicine. And since I don't know what type you have, Try any and all types of food and medicines to help. How about a manual, do you have one? It should explain how to take care of it.
