Well already saw some of the greats in the answers,Sixth Sense I personally feel is definitely at the top of the heap,my personal recent favorite are the Saw movies,First twist was better because you flat out never see it coming,but they pulled it off excellently in Saw 2 considering most are looking for the twist from the beginning,cant wait for Saw 3,and to go back to Sixth sense that's something M.Night does well because his twists in The Village and Unbreakable are pretty good too.

Yep most of these answers have great twists but for a light-hearted film with a twist try April Fools Day (1986)

LOL....definitely The Sixth Sense and The Usual Suspects! I had to watch Sixth Sense a second time to fully get it. As for The Usual Suspects, I seriously said, "Whoa!" at the end because I was THAT surprised at the twist! :-)

An old early 90's film, 'Jacobs Ladder'. The whole movie was like a Marilyn Manson video very spooky and fresh for it's time and I couldn't make any sense of it until the last scene. Good film.

'What lies beneath' has a memorable twist because the rest of the film is relatively slow and about setting mood.
'The Game' starring Michael Douglas and Sean Penn is probably the most far fetched film that purports to be set in reality ever made.
