You will never go wrong with roses, especially in her favorite color (if you don't know pick red). Do a dozen, preferably long stemmed and with a ribbon. If you pick them up they'll cost $25-$50 and you can talk the clerk into wrapping them nicely. Upscale grocery stores will carry them if it's late.
Restaurant--pick a restaurant similar to a place you went when you dated and tell her it reminds you of then. Other choice: if you went somewhere with a distinctive food (Italian, French, etc.) or you ate something unusual during a favorite vacation or your honeymoon, pick that kind of restaurant and talk about the memories.

Stop at the grocery store on the way home and get some roses. When you get home, tell her you want to take her out to where ever she wants to go, that way you don't have to worry that you don't have reservations.

Call your connects QUICKLY ( people that have have your back) plan out an evening that is a surprise to both her AND YOU, while, you are out have you people set your house up for a romantic night......Trust me I've done TWICE

a big spray of flowers and cafe rouge if you have one near you they are fantastic its french and its prices are very reasonable
