A dumb-dumb is still just dumb. In some cultures repeating a word gives it extra importance but does not change the basic meaning. If you say "he's dumb" it is simply a statement but if you say "he's a dumb-dumb" it implies that it really bugs you, not that he is any dumber.

Actually it is Dum Dum which is a brand of lollipop given out at banks. Sort of an affectionate playful way of acknowledging someone who has done something stupid it has the equivalence of dummy. For somereason if you say it twice or put a y at the end of it; it is more acceptable and less insulting.

but you are comparing a noun to a series of adjectives (comparative and superlative), which means that you can attach the adjective to the noun, and you would have a dumb dumb-dumb, or you might even have the dumbest dumb-dumb.
