Chimney repair isn't a do-it-yourself project unless you're experienced. Hire a professional.

Is the house moving or is it the chimney? Figure thatt out with a level.

You are not going to be able to do it. Get a cement contractor who has teh capacity to get under the chimney footing and pump in hydraulic cement until it goes plumb (that ain't pipes, that is when it gets straight up).

you need someone with foundation experience. Foundation is giving away and the result is the chimney pulling away. You need to hire a professional if you want to keep the chimney.

i did that once by digging under chimney & setting hydraulic jack under it, [2 jacks may do better than 1]leaning slightly in towards house. needed steel plate above jack solid footing under. pressure treated 2"x8"x?" carefully digging so as to not disturb chimney. . after carefully jacking & getting chimney close to wall, i filled hole & jack with bag of prremix concrete. mix & fill getting as wide a base as i can. threw steel in wet concrete 4 lateral strenggth
if frostheave is your problem, you can surround chimney base with 2" thick styrofoam 2' wide to prevent cold from penetrating under chimney. try to seal foam tight with canned foam. cover with poly plasic & bury foam to protect. good luck

I wouldn't do this by yourself. This can be a very serious issue and be very costly. I would get several estimates and go with the best.
