Find your muse dude.
writing is sharing yourself. dig into your joy, your pain.
now if you have the lyrics and music and just cant but them together
trial and error
play and sing. hell, play and talk. You'll feel when its right.
just go to work, learn what does and doesnt work.
figure out the sound of your voice, your song topics, how you play guitar and make sure they work and change one if you have to.

Find a book about how to write lyrics..especially if the book has a section of writing lyrics with guitar..
Here's a book for suggestion: (You can write song lyrics)..you can find this book on (Chapters.ca)..and if they don't have it in store..they'll order it for you..

You are already head and shoulders above most people! Just take the first line of one of your poems and find another phrase that would rhyme with it and work in words so it still conveys the same message.

sing your peoms and play the guitar and see if it sounds right to you or try to make words as you play the guitar and see if it fits with the sound

you'll have to start with a feeling or thought. write it as you go and then rewrite. if you have a gift for music, go with what you are feeling or thinking at the time. good luck
