What kind of ultrasound?

If it was a gynecological ultrasound, sag refers to sagittal plane, which is how they were viewing it and endo probably meant they it was the endometrium. So a sagittal view of the endometrium.

A median plane of the body divides it into right and left halves (also called mid-sagittal)-- the sagittal plane is parallel to the median. So it's just some vertically "cut" view.

Nurses don't do ultrasounds. They're done by techs. Techs aren't doctor, or nurses. They're just people who are trained to operate the machinery. They aren't qualified or even allowed to comment on what they think they might see during the procedure. "Here's the head, spine, heart, etc., should be the extent of their conversation with you. You're not the first person who's been confused or worried about the crap they've heard during their procedure. That out loud talk is completely unneccessary, and I often think it's done to make patients feel they know more than they do. Next time ask if the machine works by voice command. If they say, "No.", tell them to be quiet.
