There are a considerable amount of factors that need to be included into your scenario to accurately answer this question.

First of all you have to adequately define what you mean by "didn't eat that healthy." Is not eating healthy including alcohol, extensive caffiene, sugar, etc? Or is it simply missing a couple nutrients here and there?

The statement also seems to suggest that the second mother or the second baby has smoked throughout her pregnancy, which is highly unrecommended. No one should be exposed to secondary smoke. Significantly moreso, neonatal fetuses should not be exposed to nicotine.

The real concern of the question however seems to be drug use (aside from cigs). First of all, it depends on the drug in question, certain drugs have a tendency to stay in the human system for a considerable amount of time. So it depends when she stopped using drugs, are we talking a month, or a couple years. If she has been smoke free and drug free for say seven months prior to pregnancy (not prior to her being aware of pregnancy, but literally prior to conception) then the second child will be healthier than the first. With the assumption the second mother isn't smoking.

To conclude, if we presume that the mother of the second child had a sufficient detox period of time before conception to sufficiently cleanse her system of the drugs and nicotine then the second child will be the healthiest.

-----------------------

In light of additional information (that the mother smoked throughout pregnancy and took medication), I'd have to say that it still depends on other factors.

As the information stands right now, the healthier child would probably be born to the first mother. But it depends on the medication(s) in question, on whether or not it will pass through the cord. Additionally, it also depends on how you are defining "didn't eat healthy."

I think baby 2. My sister smoked all the time which I am not promoting at all, but here son came out totally healthy and now he is 15 and is already 6'1 and is very healthy. But on the other hand even if baby 1's mom didn't eat that healthy wouldn't the baby just take all the nutrients from the moms blood and that during the pregnancy?

i don't get it...the second mother does drugs during the pregnancy or no? or has just done drugs previously? smoking during pregnancy can cause all kinds of complications. the eating healthy part...i didn't eat the best during my pregnancy, i have to say. i was sick a lot and had ridiculous heartburn. i don't think i ate enough most of the time. it was hard to eat. i never felt hungry. the prenatal vitamins are supposed to make up for nutrients the mother misses in her food. so, i'd say baby 1 has a better chance of being completely healthy. but, really, both could be completely healthy.

I would still have to say BABY #1
I have heard of women who tried to hide their pregnancy by not eating a lot or good so they didn't gain weight. The baby's had low birth weights but were otherwise healthy. My mother smoked with both my brother & I and we were both full term, healthy weights & neither of us has any health problems. But that whole "drug" thing you mentioned, that can do a lot more damage to the mother's body, I would think.

Baby 1 would still be healthier. The morther of Baby 2 still carries toxins in her body from what she did in the past.
Your body makes up for a bad diet while pregnant if you are too sick to eat healthy or can't for other reasons. Mother of Baby 2 would be healthier post pregnancy, but baby 1 will be healthier.

Neither situation is ideal, but the drug free mom has a better chance of having a healthy baby. Babies get their nutrients from the mother's body during pregnancy, so if the mom isn't eating healthy, she's doing more harm to her body than that of her unborn child

Does the second mother use drugs or smoke during pregnancy? That's a major factor in the baby's development.

Are both moms getting prenatal care/checkups?

Are all parents having reasonably healthy DNA?

Are both moms going to good hospitals to give birth? Getting immediate care if something seems wrong during pregnancy? Getting ultrasounds and follow-up visits?

Are both moms the same age? Women 40+ have a higher risk of babies with birth defects.

There are so many things a good mom needs to do in order to maximize the chances of a healthy baby.

I think baby one because the mother didnt smoke which could have given the baby a low birth weight and the prenatal vitamins would help give the baby nutrients even though the mother didnt eat that good while she was pregnant.

I would think that how your baby comes out is a little more complicated then that. Eating healthy and being drug free certainly contribute to a baby's health, but I know a woman who had a so called perfect pregnancy and her baby had many health problems.
