The primary appraisal is only a quick view of the current issue that is plain to see. At this point you still do not know what is going on.

The secondary appraisal is when you take a look at the issue to see what is causing the issue and what the out come will be if it is not corrected and how quickly it needs to be correct.

The perception stage is part of the secondary appraisal where you start to understand the full picture of what just happened what caused it and what will happen if not corrected.

The challenge stage comes last being you now have the full picture and you know what has to be done in what order so the challenge is to limit the damage from spreading while repairing the cause and setting in place preventive measures.

But like in the second answer you could look it at a mangers level whom first instinct is to just TOTALY FLIP OUT then get everyone in a big tissy. In my above example the cause was poor planing at lower levels which the Manger should have been keeping an eye on and without creating a stressful situation requested the next level of mangers to keep an eye on. It should have never made it to the end of the month with out being taken care of. Upper management is the problem and a replacement manager is needed or training classes are needed at the higher levels. Lower levels are only tools and are doing as they are being controlled.
