Use Powerpoint.

Measure the dimensions of a DVD cover that you like and create a template in Powerpoint. Make a hairline black border around your 'live' area so that when you print you'll know where to cut.

Don't know about free, but Adobe PageMaker 7.0 has a layout which they call "Jewelcase Booklet" which just fits inside a DVD box. So if you printed those and slipped them into a clear jewelcase, the front of the booklet would become the label just by showing through.

Does that help?

Paint Shop Pro

You can put measurements in to produce a creation space the same size as your dvd cover space. It will then show a percentage of the size e.g 70% so you don't have to worry about moving the sliders around.

use proper software like fireworks or photoshop, find out the proper dimensions for DVD cases, and print them off that way!
