How much do I tip bell boys, etc. at excellent hotels?

Bell boys, the guys who get my car, anyone else? And how much is a typical tip at a really excellent hotel?

They say the standard tip is 2-4 dollars per bag that they carry. Depending on the hotel itself, you say excellent, does that mean 5 star hotel? As for getting your car for you, thats typically 5-7 bucks.
