Nobody but J. K. Rowling and her publishers know the title or the release date. The speculation on the web is that the book will be released next year, possibly on 7-7-07 (because seven is the luckiest number). But given that the next Potter movie is released the week after, many are speculating that the book may be released on the 31st of July instead, as this is Harry Potter's (and J.K. Rowling's) birthday.
The title is a closely guarded secret. We don't know.

The only thing we can say for sure is that it will be released first at midnight in the UK on a Friday or Saturday, so that kids won't lose sleep on a school night.
Personally, I reckon the title will be "Harry Potter and the Anticlimax".
