Anytime you put in "now" when asking "what's wrong?", then you will sound annoyed. If you don't want to sound annoyed, you omit the "now". Adding the "now" implies that they always have something wrong (or you are associating a ngative to that person).

The word "Now" gives the tone of annoyance...implying there was something wrong before and again now. It does not convey a sincere concern, but rather a response to a routine "dance".

Any question may be classified the way it was said, body language, tone of voice, it doesnt matter what the content of the question was....

it is always better to put the question as what is wrong now? by asking this type of question the questioner wants to know the reasons for any thing wrong. "now what is wrong?' by asking this question, the questioner is hurt and feeling there is nothing wrong and challenging the person who is supposed to answer the question to prove any thing wrong

I prefer the approach from Duke's Fuqua School of Business that Samuel L. Jackson used in one of his recent dramatic films:

"What crazy $%&&*$%$%^ let all these &^%^**&^#$$ing snakes loose on a plane? That &*^t is wrong, woman!!!"

Both insinuate or assume. I find it is most often better to ask information seeking questions like..." is there anything wrong?".

What's wrong and What's wrong now is very condescending.
I prefer to hear : What is the matter (and not what's the matter)
